I would like to order a subscription but would need to get an invoice for this please.
I also require someone to fill out a preferred supplier form so that we can make payments to you.

You can check your invoices by following our user guide after you upgrade your Jotform account: How to Check Invoices
On the other hand, I cannot understand you properly. What do you mean by requesting "supplier form"?

I can only make a payment through an invoice being sent from you.
The ’supplier form’ is a form that allows us to make that payment to you, or is that not something you would do?
Iv attached the form. Maybe that would clear it up.
