The phone numbers are not showing up on my submitted applications. I have tried deleting the fields and replacing them, I have change the code many time, and now am trying and Iframe. I have tried turning on and off, then off and on...ect.. What is going on with the phone number fields? Can you go into my dashboard to see if I need to do something else.

I'm sorry for the trouble caused to you.
I did check your form's notification email alert and found that the field name of the phone number is different. It seems you have edited your email alert and then deleted and added the form fields again.
Please note that if you edit your email alert, it becomes dirty. That means after you edit your email alert, any changes you make in your form will not reflect in your email alert. The best way to solve this problem is to delete the existing email alert and add again. A new email alert will always include all form fields of your form.
