"Image Upload Preview" is not being displayed in IE browser.

we have a submit image filed on our form. the submit image field and button do not show up on Internet Explorer

I'm sorry for the trouble caused to you. 

I did test your webpage where you have embedded your form and I am able to replicate the issue you are having. The "Image Upload Preview" widget does not gets displayed in IE browser. When I tested your form with its direct URL, the widget seems to get displayed in IE without any problem. 

I am not sure what is causing this issue but most likely some script of your webpage is conflicting with the scripts of your form. I would suggest you to please re-embed your form using its iFrame embed code and see if that solves your problem.
