at the end of the following form (http://form.jotformeu.com/form/22113318791348) a thank you page is supposed to be shown. I don´t know why this does not work! There is just a blank page shwon instead of the prepared thank you page.

Thank you! It is getting more and more urgent since the form is supposed to go online today! Furthermore, I also do not receive notification emails and no autoresponder mail is sent. Thnak your your help!

I have cloned your form and was able to submit successfully. So please try to clone the form your end and see if it helps (use the clone). See link below for a guide.
As for notifications and autoresponder, please try to remove and re-create both using clone form.

thank you for your reply. I did what you suggested and now at least the autoresponder mail works. I´ll wait a few more minutes to see whether I´ll also get a notification mail.
Maybe, one reason mitght be that I have a second account (an old one) with the same mail address registered as the one that is now used for the notification mail.
The name of the account is ChristianP. If possible you can delete that account!
