How can I make my question field go all the way across when sent through email?

We have an online application form that we are trying to get on our site, but are running into issues with the last question. It is the simple YES/NO that all applications have, such as 'I understand that, under state law...etc.' and it looks fine on the application form itself, but if we or anyone fills it out and it gets emailed, the question is only in a 1-column state and it's very stretched out (examples below). I have went through the edit screen and made it 4 columns and more, and have tried everything and we're not sure what the next best option is?
