How do I add sub fields

HI if I wanted to create a form such as adding subfields under the selection as see on the URL page below.

When you select a field under choose issue, I would like to add a sub form or fields under each of the following :

For example: choose Faulty Product Received brings up

Defects, select Product defect, Discoloration Material damaged

Upload image of Product

Action Reguired, Select the option you would like

Request Fast easy Exchange

Request Product Refund
