Evidently the SUBMIT button is at the top of my form.

I checked your form (https://www.jotform.com/71084713012142) and the submit button is indeed at the top of some fields. To place the submit button and other fields in their designated places, simply drag and drop them as shown from the screencast below:

The submit button looks okay now but I didn't change it, did you? It's
hard to trust that when I send it to a client that all will be well. Also,
before writing to you I changed the name of the summit but it isn't
reflected on the form. Can I arrange for a call?
