I want patients to sign 5 forms using a usb signing pad. Can you help?

    Currently, out patient input system has us print 5 sheets which patients sign. Then they are scanned in to their file, and we shred the paperwork. I want to use Jotform to create the forms and  have them signed somehow. Do you have a solution in place?

    Hello Brian,

     

    You can use this widget to help you with your problem.

    Just add it to your form and it should do the trick.
