My form is disabled

I have one jot form called RES rental application. However the form is disabled and I can't enable. Can you please assist me. Thank you

The account was suspended due to the imported form in your account which is already deleted. I have now activated your account and you should be able to view the form normally. However, I also notice that there are few fields that you are collecting Bank account information which is considered as sensitive information which might cause the account to be suspended again. There are few guidelines to collect the bank account information on the forms and we request you to go through the thread below that can help with it.
