PDF form formatting: text is overlapping, how do I fix?

I created a form, and tried to export to pdf, but the text is overlapping https://form.jotform.us/johnchandler32/locationshawaii-website-survey 

How do i fix this so I can print the PDF and its readable?

You can customize the layout of the PDF report from the submission view page.  Here is more information on how to do so:
