How can i create a credit card authorization form for my website?

    I've tried a few times to create a credit card authorization form for my website, so my clients can use that instead of having to scan and email the document back to me. I've gotten locked out of my account twice now. Please help me get this figured out.

    I have found only account with username Jkirby10. That account is autosuspended by our phishing system. 

    It is not allowed to collect credit card info on plain text fields.

    Terms of Use

    I have inspected forms that you created on that account and I saw that you are using our payment tool, so I am not exactly sure why your account was suspended. It is most likely because of the words "credit card" in heading of your forms. 

    I have removed "Credit Card" words from the headings of your forms and I have activated your account.

    I would also suggest you to change the title of your form CC Auth.
