For some reason, the information captured in the PDF either appears in a random order or info is missing.
Why would that be the case?

Please try resetting your PDF submission design: https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report
Then delete your notification, and create a new one: https://www.jotform.com/help/25-Setting-Up-Email-Notifications
Make sure to enable the PDF submission attachment:
