    Your account and forms were suspended due to violation to our Terms of Use.

    Scams. You agree that you will not use JotForm or any form(s) you create using JotForm for scamming purposes to attract, lure, or illegally obtain payment of any sort from others by unjustifiable means such as posting a jotform as a money-making program on a classified ads site as an example of that. Any account reported or found doing so will be immediately suspended.   

    Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    You were using our form to collect credit card account data.



    Paying for the subscription will not reverse the suspension of your account due to violation. We have cancelled your payment.

    You will have to delete the illegal form and any other similar forms on your account before we can request for consideration to reinstate your account.

    First of all, I want to apologize for this. My name and my reputation are my biggest assets which I do whatever it takes to preserve. I assure that never did and will never do any wrong in this matter of scam/phishing.

    At the very beginning I should have used this form once, but if you check my usage you'll see that all my payments come through Paypal and from a couple of forms I created myself, which do not request any credit card information.

    I'll be more than happy to delete any form that does not comply with JotForm policies, I just don't have access to my account as of now.

    Please reconsider your decision and reinstate my account. Also, subscription payment can be done as early as today.

    I have removed the form with issue from your account/username durvalpinheiro

    and reactivated your account. You should be able to login to the account now to review the updated status.

    Please make sure to double check your forms again and immediately remove any similar form to avoid getting the account blocked again for the same reason.

    You can upgrade your account anytime should you prefer to.
