I haven't seen a spot where I can delete my account or is that something I have to request?

Only support staff can delete your account. Before deleting your account, please be advised that once it is deleted, you could no longer access your form or view it.
I checked your account, and there was no form created on it. For security reasons, we would like to ask for your confirmation if you wish to continue with the deletion of your account.
