I'll paste an image of the screen shot seen this morning. I have web access to the end user, but I have no idea who created this form, not how to fix whatever has stopped if from working. Please advise quickly! Thank you.

I checked your account using the email address you used in posting up the inquiry. I'm afraid I could not find any active account or forms under it.
Do you have any other Jotform accounts where we can retrieve your information? So that we will be able to know the reason why you are getting the error.
Anyway, there are two probable causes of that one, either you hit the Submission limit or you have reach the allowable forms allowed in your account.
Either way, you can try to delete submission or archive a form to make your form active again. Or, you may try to upgrade your subscription.
