One page of the form is not mobile responsive

The form looks great on a phone except for page 4.  I have a text field there and that's probably why, but I can't figure out how to limit the width of the text field.

Your help is appreciated.

I was able to replicate the issue. This is happening because of the E-signature widget on page 4. It seems that it is breaking the alignment or adjusting the main width of the form.

Please remove the E-signature widget and replace it with Smooth Signature widget.
