    Form not working

    I was referred to your site by a developer friend and really like it. Want to upgrade, but the first form I designed does not appear to be working.

    I'm not receiving the form submissions, although test emails come through just fine.

    The other strange thing is that after a form submission, the redirect goes to a totally different domain's home page that appears nowhere on the form.

    Here's the URL where the test form is published:

    I have checked your account's mail logs and I only see successful test email. I have checked your form's submission and I did not see any submissions in there. If you want to test your form, please make an actual test submission directly to your form. (Fill up your form and submit it after)

    Also, I see that you have set an external link that will serve as your form's Thank-You page when the form is successfully submitted. Redirecting-Users-to-a-Different-Page

    May we know what exactly is the home page you are referring to, please?

    With your permission, we will be glad if you will allow us to make test submission(s) directly from your form on your website. This way, we will be able to determine the issue easily.

    Thank you for the reply.

    Unfortunately I mistakenly tried replying to the email I received rather than here. Duh.


    I have gone to the temp page where the form is published, have filled-out the form, have checked the reCaptcha, and have submitted the form. I’ve done that several times.


    The URL of the temp form is: www.myvalparaisofamilydentistry.com/contact-06-16-17.html


    I have set up a redirect to the Home page of the site where the temp form resides.


    The URL to which the redirect is supposed to take the visitor is: www.myvalparaisofamilydentistry.com/index.html


    But after I fill out the form and submit, I am taken to the Home page of my own company’s website?


    That URL is: www.trinityinternetservices.com


    I have double-checked my JotForm settings, and unless I’m missing something (which certainly could be), everything there is correct. When I do a form test from the JotForm site, I do receive the test, but when I test the actual form I don’t receive anything, and am re-directed to the wrong page/site.


    I’m at a loss what to do from here, so any help or suggestions you can offer will be greatly appreciated.

    I would like to apologize for any inconvenience this is causing you. We appreciate you explaining to us the main issue(s) you are having and we will be glad to help.

    I understand that your form resides on this website, www.myvalparaisofamilydentistry.com/contact-06-16-17.html

    And you are trying to redirect your respondents to this website when they successfully submitted your form. www.myvalparaisofamilydentistry.com/index.html

    However, when you tried to make test submissions (from your website), the submissions were not recorded on your form's submission page and you were not redirected to the page you set as your form's thank-you page.

    We can try to make test submissions on your form on your website and also from its direct link with your permission. This is one way for us to determine what causes the issue you are having.

    Meanwhile, I would suggest you try to check if using a different browser would still result in the same issue(s). You can also try to clear your current browser's cache before you make another test. Also, I see that you have embedded your form using its embed script, can you try if embedding the form using its iframe embed code would resolve the issue?

    It works on Firefox and MS Edge, but not in Chrome. Any suggestions?

    I checked your embedded form on your website and I found that it was embedded using the script embed code.

    There could be script conflict issue happening since there were also other jquery script codes on your website. As suggested by our colleague can you please try first re-embedding the form on your website using instead its iframe publish code.
