How do I make a single choice WITH a fill in the blank

I am trying to do something similar, a single choice WITH a fill in the blank: see example

A) Benefits begin the___ day of the month following___  days of eligible employment.

B) Benefits begin the first day of the month following___ months of eligible employment.
