    Why do all my forms say form not found, forms are disabled?

    Dear Sir,

     

    I have created the form but its not opening and showing form is not found , or disable

    Your account was disabled by our anti-phishing system because of this form http://www.jotform.me/form/71761035585459 

    And the fields that triggered the system are:

    It is against our Terms of use to collect bank information and any related data.

    I removed the affected fields from your form and re-activated your account. 

    Please avoid putting them back so your account won't be suspended.

    Ok, I would assume my problem is for the same reason. I didn't read all the terms....sorry about that. Can you please unflag my account to create a new form. Thank you!
