The owner of this account is leaving the company, and we would like to change the ownership, while keeping all of the previously completed forms and submissions.
is this possible, and what would be required?

You can change the email settings and even the password of your account by going to the account settings page. You can find the button to account settings by clicking My Forms at the top of the page, and then Account settings button at the lower right part of the same page.
Furthermore, we can also change your username for you. Just let us know if you wish to do this.
