I just received an email that my password was changed - is this legit?

The email said it was changed on Thursday 6/22 by someone in Germany? Please see attached screenshot and get back to us. It looks fishy.

Sorry for the inconvenience. A user who uses JotForm has been sending spam messages to some emails. This is a phishing attempt. we have already taken action by suspending the user's account.
