What does ''clone of event request'' mean?

we have a form where patrons can request details about events and renting out space for private events. I received a form that said "clone of event request" - what does that mean?

I have checked your account and found that, among the forms you have, there are two forms named "Event Request" and "Clone of Event Request" respectively.
