My heading image is not on the form when it is emailed back to me

I understand that the header image is not displaying in the notification email. You may insert the image by uploading it manually to the email by clicking on Insert Image button. Please see the screenshot below:

You may also try creating a new notification so that the header image should be inserted automatically in the email.
