How can I integrate more graphics into my form / boost conversion rates?

I am running a facebook ad campaign that funnels people to my form:

I am having a fair amount of hits, but a very low % of conversions.
I believe that sprucing up the form with some images, specifically on the right side of a long column of question fields might help.

Please help me to understand how I can place more graphics throughout, without destroying the format of my form, as well as any advice on how to boost the conversion rate (so far out of 30 people its 0%).
