When the customer receives their emailed confirmation and copy of the form, the logo is not there and when I cread a PDF fillable version it is not their either.

In your email settings for your autoresonder, there is an option to add an image. Click the image insert image icon, then upload your logo image.
From there, you can resize it to fit on your email. Just like this.
