My account is autosuspended. Please help

It tells me to autosuspend when I log into the account and the forms do not work for me

I have checked your email address and see that there is an account evencs1 registered using this address. This account is active and good in standing. However, when I check the IP address I am able to find another account evencs. I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I notice that two forms in your account are collecting passwords. It is not allowed to create login forms or collect sensitive information like Credit card numbers, passwords, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.  

In order to activate your account, it is required for you to agree to our terms of usage and to delete the guilty fields on the form. Please confirm so that we can delete the guilty fields in the forms and activate your account.
