I'm having trouble permanently changing the name on my forms

I used a cloned form as a starting spot for my online checkout form.  I changed the name to save it and it appears correct in my list of forms, however when I copy the link the header still reads Bare Bones Form for Team Members.  How can I get rid of that so that it reads the way I want my clients to see it?

You simply need to change the page title from settings, please click on SETTINGS > FORM SETTINGS > SHOW MORE > and select PAGE TITLE
