Why is my form disabled?

    I have checked your account and found that it is autosuspended, because you have some forms in the Trash folder named "Direct Deposit" and these forms triggered our automatic form review systems due to suspicion of violating our terms of use.

    As these forms are deleted by you, I would suggest purging them from the trash folder so they are deleted permanently, or granting us permission to do so on your behalf, in order to re-activate your account.

    Gotcha.  I thought that had already been purged a month or so ago.  It's purged now.  Can I get back up and running now?
