Once sent I have now started to receive two forms in my email.
I noticed this yesterday.
Please complete & submit the form then check my account.

The reason for this is because you have two email notifications setup in your email settings. They are both addressed to you.
You should delete one of them. And then you will receive only one notification when your form is submitted.
