i want to log in and I can't, because of "Login failed - AUTOSUSPENDED"

Our anti-phishing filter tool automatically suspended your account because of the word "Login" and "Password" in the Header Text field. Here's a screenshot:
We need to delete the word "Login" and "Password" to your form in order for us to re-activate your account back in good status.
