Is it possible to use form field-text on the documents in Docusign?

I'm considering using Docusign but I need to know if you can use the form fields for auto text in the documents that the user signs in Docusign?

The docusign widget only collect the signature, full name and email address as a part of the widget.  Any further text will be captured in the form data, but only the above data will be forwarded to docusign.

is it then possible to somehow combine the normal e-signature field with
the text field above on page 5 in my form? It would be great if it could
somehow export a pdf with only those two fields..

I have tried cloning the form to my account and added DocuSign widget to the form. As I tried to sign the form in Docusign after submitting the form, the text is displayed on the document along with the e-signature field. Please see the screenshot below:

However, the signature captured through the DocuSign is also displayed in the signed document.
