I need assistance gaining access to my account. I am afraid I don't recall my password and might be remembering my username incorrectly too. Any assistance would be greatly appreciated.

Upon examination of your account, I found that it is indeed suspended, because one of the forms you deleted was collecting credit card information.
In light of this, it is a must to take any of the following options:
1. Integrate your form with any of our Payment integrations: https://www.jotform.com/help/323-Mastering-Payment-Form-Integrations-with-JotForm
2. You can take advantage of a feature inside of most of our payment integrations called "Authorization Only" which allows you to collect payments manually at a later time. Here is a more detailed guide on how to do this: https://www.jotform.com/help/292-How-to-Enable-Payment-Authorization
As the form was already deleted, I have purged it from your account and then re-activated it. Your forms should be working properly now.
Regarding the account access, I have checked your email activity and found that several password reset instructions emails were sent successfully to your email address. Please check the SPAM/JUNK/BULK folders in case they're landing there instead of the inbox.
