How can I create a multiple choice with a text box?

Looking to set up the following question:

Are you planning to continue your research in your SURF field of study?  If so, how?

Options:

Special Studies, Please specify your faculty advisor

Honor's Thesis, please specify your faculty advisor

Other, please specify

I am not going to do any more research on this topic, because....

Use our Single Choice field then enable the Other in the field settings.

You are currently using our Dropdown field, but this is not possible in this field. You have to use the Single choice.
