Multiple entries to add and send the value for payment

Hi - I would like to use Jotform for our church giving page.  But, we need more than one user defined field.  As of now, I can only use one.  For example, I need a blank where a person can enter their desired amount for tithes, than another for offering, then another for missions, ect.  Is this possible?  If not, I'll have to use another form creator. :-(  Thanks!

I have checked the web page shared and see that multiple fields are added for payment. Though adding the fields to the form is possible by using a configurable list widget, unfortunately, it doesn't support calculations. However, you may consider adding multiple fields on the form and setting up few conditions to display them.

You may then perform a calculation using the calculation widget and send the value to payment field to process.
