I am not sure if I understood your question correctly.

Do you want to password protect your form?
If so you can add one field at the top of the form. You can name is "Access code". Then you can use conditional logic to show all other field on your form if access code is correct.
You can set your condition like this.

Access code is 12345
Do not use Password word, because that will suspend your account.
