Conditional autoresponder is not working

I do not receive a notification when I register to pay by check. I've used this condition on other forms and cannot figure out why this is not working.

I have checked the conditions and see that they are set correctly. As I cloned the form, I am able to receive the email normally when the check is selected. Could you try adding a new notification to the form and try changing one of the conditions with the email to the notification to check if that works?

I have tried cloning the form and was able to replicate the issue at my end. As I tested the conditional emails on a separate form, they worked normally. After investigating the issue on your form further, I see that it was being caused by the field 'Preferred Payment'. As I tried testing the condition with a different field on the form, the emails were sent without any issue. I have tried editing the options in the radio field (removing and entering the same options) and set the conditions again which worked fine. 

I have made the same changes on your form as well and the conditions are working now normally. As I tested it, I have received the email based on the condition.
