My "Request an Account" form does not complete. You fill everything out and then when you click to submit the "thank you" image doesn't show up. And the e-mail is never sent as designed.

We would like to apologize for any inconvenience. I was able to reproduce the issue you're having with the thank-you page on a clone version of your form. To fix that issue, I suggest you try to make changes to the default thank-you page and save the form. After doing that, you can revert the changes you made by editing the thank-you page again. Please test the form after.
With regards to the issue of not receiving an email notification after a successful form submission. I found out that the email address set as a recipient of the notifier in your form is on our bounce list.Result: email@example.com is IN the bounce list
Reason(s) : X-Postfix; unable to look up host
I have removed the email address from the list and you should start receiving emails again to that email address when your form is successfully submitted.
