The email notification is coming from email@example.com and not the stated email which is a selection in the form. How do I fix that?

Please go to Email Alerts > Notification > check if the Reply-to Email is set to your email address field. The field name may differ depending on how you name it on your form.
After which please test your form using the direct form url.

I got it to work. Changing KEEP MEDIA2 as a Hidden field with an email address as it default value would sort it. Below is the cloned form, you can clone back it you want to check.
