I just created a form with a survey and for some reason there is a white box around the choices that I can't get ride of.
Can you tell how to, thanks

I cloned your form and I found out that there's an inline CSS for each row item in the Input Table. The inline CSS contains "background-color: rgb(255, 255, 255);". This is the one causing the issue, please remove it.
Just select the Input Table field and then click the "Properties" button. After that, go to the "Fields" tab and delete all "background-color: rgb(255, 255, 255);" string.
Here's the result:
Hope that helps. Thank you.
