Embedded form not showing on website

    I have created a form on jotfom but it is not showing on my website.

    Your form is not showing on the website where it is embedded because the link in the "src" attribute of the embed script is not correct.

    May I suggest you re-embed your form using the embed script that can be copied from your form's builder page under Publish?
