I see your website is created under Wordpress.org? If so, please try to re-embed your form by using any of the following below: 

Method #1: Using the iFrame embed code: https://www.jotform.com/help/148-Getting-the-form-iFrame-code. 

Method #2: Use this Wordpress plugin instead: https://wordpress.org/plugins/embed-form. I see you are using the JotForm oEmbed plugin, try the other one I mentioned.
