Please tell me, is this possible with JotForm?
I offer printing services. I would like to create a form for my customers to order business cards. They would input all of the normal info as well as a file upload... this I know is easy. But, I would like to have 2 additional features
1) the ability to click a "Next" button for shipping options for them to choose (trying to keep the form tidy)
AND 2) Since many companies have multiple employees and will be ordering the same business card, but have multiple employee names, I would like the ability to have a button that will allow them to "Add More Names" which would be all the same fields collecting info but for as many additional people (employees) as they need.
Please advise if this is possible, thank you very much!

Thanks for asking.
Inserting a page break would take care of No. 1:
Unfortunately, providing a button for users to click if they want to duplicate a field or set of fields (similar to the demo here), though requested by more than a few users, is not yet a reality.

Could I use some type of conditional field (in a conditional field) to add more personal info? Like a button that says "Add additional names" then have the name field appear... with another conditional button that says the same thing? So maybe I set 10 additional name fields?

Yes, that can be done. I'll create a demo form later to illustrate the solution.

Okay, here's one way to do it. The best way to understand how the form's conditions are setup is to clone it and comment out or delete its injected CSS before viewing its conditions. The conditions will still work if the labels are renamed. They were only labeled in that manner (Referral 2, Add Referral 3 and so on) to make it easier to create the conditions.
