Trouble inserting a pdf file

    Hi - recently created a form with two pdf files attached.  Found a typo in one file and had to remove it, edit it and then tried to reload it.  The reload shows a 404 - file not found instead of the document preview with pop-out box.  Then tried to copy the pdf file that was already in place on the form and tried to edit it by uploading a new file - same error 404.  Trying to figure out what is happening here...

    I checked your form and the PDF is showing on my end:

    I am also able to view the PDF from the pop-out. 

    If this is not the case on your end, let us know and we will take another look.  As far as I can tell though, the PDF is loading in the form.
