I have two "If's" in a conditional logic ("if ________ is selected" and "18 or older?") and under those I have "if ALL of the if's are equal to ______" but when I go to preview the form, I can select "yes" in 18 or older (which is only ONE of the "If" statements in my conditional logic) and it shows the addition on to my form that should only show up if I select "yes" for 18 or older and also click on a specific choice. How do I get it to apply both of the conditional logics (note, I have already selected the "if ALL the statements..." option)? Thank you!

If you select the Form Collapse that only shows if the condition rules are met from the conditions search drop-down of Conditions in your form builder page settings, you will see that there are three conditions associated with it. One of which is a duplicate of the other.
I suggest you delete the duplicate condition because it creates conflict. To delete the condition, click the cogwheel icon and select delete from the options that will appear.
If you open the duplicate condition, you will see that there is a condition rule (if) that has no selected value.
