Why does the form show up in some Chrome and Firefox browsers, but not others?

    May we know what internet browser that your form is not working? I checked the Page URL you have provided using my phone (Safari Browser) and it is working properly. I also did a test submission to your form and it was submitted successfully. Here's a screenshot:
