Conditions not working on form

    This form is set to display a different fee (referred to as Prorated Membership) on/after July 1 of the year. But if the applicant selects "Family", the form is supposed to show several new fields to capture information about the second applicant (name 2, email 2, etc.).  However, it's NOT displaying these additional fields. I have checked the conditional statements and they are correct.  I've also checked the form in Chrome & Firefox browsers and the problem exists in different browsers.

    I contacted Jotform several days ago about this glitch using your Send Feedback button on the form page but I never rec'd a reply.  Perhaps the Send Feedback button is not working as well??

    The following condition is conflicting with your Show condition.

    In general, when you apply the Show condition, you do not need to add the corresponding Hide condition.

    It is also possible to mark the fields as required on form instead of using Required conditions. The system will understand that the field is only required when it is displayed.
