    Can I upload my own form that I created?

    May I ask what is the format or the file extension of the form you're planning to upload? 

    Unfortunately, we don't have a tool that can convert a PDF or Excel form to an online web form. This tool is currently not working. Also, this tool was created for Adobe FormsCentral users.
