Form not found/disabled error message

    Hi there. I had some issue last week with my account being suspected of "phishing". I wasn't sure why, and no one ever contacted me about it. However, I was able to log on this morning. When I went to my website to see if my form links were working again, they don't appear to be. I made two membership applications for our Association's website. I tried deleting and recopying the links for the forms, but it doesn't seem to be working. If someone could just let me know how to fix this so we can actually use the membership applications, I'd really appreciate it. Thank you!

    I have checked your account and see that the forms requesting credit card information using plain text fields. Unfortunately, it is not allowed to collect credit card information on plain text fields using JotForm. Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.

    In order to activate your account, it is required for you to agree with our terms of usage and agree to delete the guilty fields on the form. Please confirm so that we can delete the guilty fields from the forms and activate your account.

    Oh. I didn't realize - my apologies. We don't use things like PayPal or anything, as we're a very small Association. Is there a way to collect that kind of information for office use? If not, please delete the offending fields, so that I may edit the form for a "please contact the office to make payment" if that's alright instead.

    Unfortunately, there is no workaround to collect credit card fields on the form other than using payment integrations. You may mention on the form to contact the office for payment. We have now deleted the guilty fields on the forms and activated your account. The forms should be displaying normally.
