My account was auto suspended? Why? Can I regain access. I just spent about an hour creating a form to have it be suspended. I am launching a startup as a social media marketing agency and already have several clients. I am trying to collect all the information I need, along with payment in one transaction.
Please advise as soon as possible.

Apologies for the inconvenience this has caused you.
Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
I have now deleted the guilty fields on your form and re-activated your account. Please refrain from doing this because it will be difficult for us to re-activate your account in the future once your account gets suspended again.
