My form is disabled?

    can you please help me as my form is suddenly disabled

    When I have checked your Jotform account, I see that it has been suspended by our anti-phishing system. You are requesting some private data in plain text.

    The password field is against to our Terms of Use. Here is an excerpt from the terms of use:

    I have deleted this field from your form and I have also reactivated your Jotform account.
